My hubby is so creative and loves making special hand-made pieces that he knows will be well-loved. This past Christmas, he took an antique hymnal and created a beautiful masterpiece for his mom!
I have asked him to assist me in creating a very simple tutorial that will assist you if you decide to tackle this project for someone you love.
I am still amazed at how this turned out - I'm not sure I would have been able to do it!
Hymnal Folding Tutorial

Begin by counting the number of individual pages in the hymnal. Using a Photoshop-type program, create a template that includes your chosen word superimposed over bold printed lines - the same number of lines as number of pages in the hymnal.
Use the template as a guide for folding - matching the size and thickness of each letter.
(In my opinion, this is where some level of genius plays a part.)
For each letter, create folds above and below where the letter shades out that line.
(Did I mention that I don't think I would have been able to do this?)
I have seen a variety of similar ideas on Pinterest, but I'm not sure any of them have turned out as beautiful as this one - but I may be a tad partial...
